Dementia and drug use cause paranoia. That is very common. Some of other things that can include delusions and/or hallucination can be:
Parkinson's disease
Brain Tumors
Grief (yes, sadly)
Migraines
Sleep disorders
Infection
Pharmaceutical drug (abuse or side effects)
